Vision Insurance Plans

EyeMed vision plans are not available for employees in Hawaii, Puerto Rico, or the American territories. 

Nova Southeastern University/ICUBA offers vision care coverage through EyeMed. With EyeMed you can enjoy comprehensive vision coverage, including annual eye exams, frames, and contact lenses. With a vast network of providers, including independent eye doctors, popular retail stores, and online options, you have the flexibility to choose the best care for you. Plus, enjoy exclusive discounts and expert support to make the most of your benefits.

NSU/ICUBA members can choose from two plan options:

EyeMed Base Plan

  • Annual eye exams
  • Frames every 24 months
  • Contact lenses every 12 months ($100 allowance)

2024-2025 Base Plan Premiums

  • Employee Only: $4.74 monthly / $2.37 biweekly
  • Employee + Family: $12.15 monthly / $6.08 biweekly

EyeMed Buy-Up Plan

  • Annual eye exams
  • Frames every 12 months
  • Contact lenses every 12 months ($130 allowance)

  • Employee Only: $7.38 monthly / $3.69 biweekly
  • Employee + Family: $18.87 monthly / $9.44 biweekly

Note: Vision insurance from EyeMed will be updated on Thursday following your new hire enrollment or the approval of the life event by the Office of Human Resources (e.g. A life event approved on Friday will be updated with EyeMed before the close of business on the following Thursday and your benefit election will be active in their system on Friday morning [actual effective dates of coverage are based on the life event documentation submitted]).
